Accurate body measurements?

There have been several threads where there have been tips and tricks to make sure you're measuring accurately, but I was hoping we could get them all in one place. I'm horrible about this and would love to hear your tried and true methods and tips. Please share.

What I've been doing is holding the tape just against the skin, not firmly and not loosely. I relax my muscles. I've seen recently that one should exhale and tighten the muscles. This makes quite a bit of difference in my belly and chest measurements, so somebody, please enlighten me! Also, when is the best time to measure? Is it like weighing, for example, the first thing in the morning, as empty as possible?

I'm a total newb, so bare with me!
