Low Carb Diet

I did the low carb diet last year and lost 65lbs in about 3.5 months. I have been on the yo-yo diet since then and I am now getting strict on the low carb diet again. Problem is I have been in Ketosis for the past 3 weeks and dropped about 9lbs total, which is for me slow but great.
I started eating low carb bread that has 13g of carbs and 12g of fiber per slice. I ate 4 slices for dinner last night and today found myself kicked out of ketosis. Can someone let me in on this and tell me how that happened? Is the bread really not 1g carbs per slice?

I'm begining to get really frusterated with this diet this time around. I started at 305lbs on April 1st. and today I weighed in at 298lbs and I was down to 295lbs a week ago. I have been eating less then 20g of carbs a day and I have also been doing 3-7 mile long hikes in the mountains along with 3 days a week on my total gym. What is the deal? Has anyone else had this kind of situation happen to them or know of a valid reason why my weight is bouncing around so much?
